Commit Pending Page Action (M2M)

/v1/bundler/conversations/{conversation_id}/client-actions

Atomically stores one model turn's transcript rows and the pending page action they requested. A repeated call_id from the same run while its action is still pending answers 200 with outcome duplicate and writes nothing; any other row already carrying the call_id is a 409 call_id_reused and writes nothing; a live pending action for a different call is a 409 action_pending. Messages without exactly one tool_call row matching the action's call_id are a 422.
